First Trimester Ultrasound
Milestones & Scan Guide
What sonographers look for as the embryo grows from a microscopic 2 mm pole to an active 84 mm fetus. Track normal fetal heart rates and anatomical landmarks week by week.
Week-by-Week Ultrasound Anatomy & Heart Rates
Clinical sonographic criteria used by the British Medical Ultrasound Society (BMUS) and American Institute of Ultrasound in Medicine (AIUM).
Week 5 (4w 0d – 5w 6d) — Gestational Sac & Early Yolk Sac Detection
The earliest stage of ultrasound confirmation. A tiny fluid collection (gestational sac, 2–5 mm MSD) appears embedded in the thickened endometrium (decidual reaction).
Key Ultrasound Findings:
- ✓ Double decidual sac sign confirming intrauterine pregnancy
- ✓ Yolk sac appears at MSD ~8 mm to provide embryonic nourishment
- ✓ Crown-rump length is generally too small for reliable dating calipers
Week 6 (6w 0d – 6w 6d) — First Definitive Embryonic Pole & Cardiac Flicker
The embryonic pole becomes distinctly visible adjacent to the spherical yolk sac. Rapid rhythmic cardiac contractions confirm biological viability.
Key Ultrasound Findings:
- ✓ First reliable CRL dating milestone (minimum ~5 mm pole)
- ✓ Cardiac activity detectable via transvaginal sonography (TVS)
- ✓ Neural groove closes; head and caudal tail fold become distinct
Week 7 (7w 0d – 7w 6d) — Limb Bud Sprouting & Cerebral Vesicles
The embryo doubles in length. Paddle-shaped arm and leg buds emerge from lateral body walls, and fluid-filled brain vesicles become apparent.
Key Ultrasound Findings:
- ✓ Upper limb buds clearly visible before lower limb buds
- ✓ Diamond-shaped rhombencephalon visible in primitive brain
- ✓ Heart rate accelerates toward peak velocity (~160 bpm)
Week 8 (8w 0d – 8w 6d) — Physiological Gut Herniation & Early Movement
The embryo exhibits tiny spontaneous flutter twitches. The rapidly elongating intestinal loop herniates naturally into the base of the umbilical cord.
Key Ultrasound Findings:
- ✓ Separation of digital rays in hand plates
- ✓ Physiological midgut herniation (normal sonographic finding)
- ✓ Fetal heart reaches maximum gestational rate (160–175 bpm)
Week 9 (9w 0d – 9w 6d) — Transition from Embryo to Fetus
At 9 weeks 0 days, the embryonic period concludes and the fetal period officially begins! The caudal tail is completely absorbed, and distinct facial profile develops.
Key Ultrasound Findings:
- ✓ Distinct fetal skull calvarium with bilateral choroid plexuses
- ✓ Elbow and knee joints clearly flex and extend
- ✓ Umbilical cord vessels (2 arteries, 1 vein) identifiable
Week 10 (10w 0d – 10w 6d) — Organogenesis Complete & Amniotic Swallowing
All primary organ systems have finished basic structural formation. The fetus begins swallowing amniotic fluid and producing early gastric secretions.
Key Ultrasound Findings:
- ✓ Distinct nasal bridge and orbital sockets
- ✓ Fingers and toes completely separated without webbing
- ✓ Clear visualization of stomach bubble and urinary bladder
Week 11–13+6 (NT Window) — First Trimester Combined Screening & NT Assessment
The most important screening milestone in early pregnancy. Sonographers measure Nuchal Translucency (subcutaneous fluid behind fetal neck) between 45 and 84 mm CRL.
Key Ultrasound Findings:
- ✓ Nuchal Translucency thickness calibrated against CRL
- ✓ Nasal bone presence/absence documentation
- ✓ Upper threshold for CRL pregnancy dating (transitions to BPD/FL at 84 mm)
How to Read Your Ultrasound Report: Common Abbreviations
Demystifying common acronyms found on your sonogram printout or hospital radiology chart.
Straight-line measurement from top of head to bottom of buttocks in millimeters. Gold standard for 1st trimester dating.
Age of pregnancy in completed weeks and days calculated from last menstrual period or ultrasound.
The projected delivery date corresponding to 40 completed gestational weeks (280 days).
The cardiac contraction frequency measured in beats per minute (normal range: 110–160 bpm in 1st trimester).
Average of 3 orthogonal dimensions of the gestational sac before an embryonic pole is measurable.
Circular hyperechoic ring within the gestational sac supplying early nutrients and blood cells to the embryo.
Fluid pocket at the posterior fetal neck measured precisely between CRL 45 mm and 84 mm to screen for Trisomy 21.
